Objective: A review of published literature was performed to assess the safety of insulin lispro use in pregnancy. A secondary objective was to report on any efficacy findings.Methods: The National Center for Biotechnology Information (NCBI) Entrez Database PubMed (http://www.ncbi.nlm.nih.gov/pubmed/) was utilized to search citations from MEDLINE in the November 2009 timeframe that contained safety data on the use of insulin lispro in pregnancy.Results: A total of 25 publications (1,265 pregnancies) were identified from MEDLINE with relevant information and were included in this manuscript. No statistically significant differences in the rates of congenital anomalies associated with insulin lispro use during pregnancy were reported, as compared with human insulin. Compared with human insulin, insulin lispro was reported to result in improved glycemic control as demonstrated by lower postprandial glucose and HbA1c levels.Conclusion: The current review of the published literature supports that insulin lispro is a safe alternative to human insulin with similar perinatal outcomes and potentially improved glycemic control in the management of diabetes in pregnancy.
